FIV in Cats: The Scariest Thing About It Is the Nickname

FIV spreads through deep bites and barely at all between calm housemates: zero transmissions in the landmark study. The honest prognosis, tests and vaccine story.

, feline immunodeficiency virus, is best known by its nickname, "feline AIDS," and that nickname has probably cost more cats their lives than the virus's own biology ever demanded. Here is what the evidence actually shows: a virus that spreads almost exclusively through deep bite wounds, that failed to transmit even once in the landmark study of infected and uninfected cats sharing homes for years, and that leaves many carriers living out an ordinary lifespan.

The nickname is not entirely wrong, which is why it survives. is a lentivirus, biologically related to HIV, and in a minority of cats it eventually reaches an immunodeficiency stage that veterinarians do call feline AIDS. But a positive test is not that stage, and may never become it. Texas A&M's veterinary college describes cats living for years in the quiet phases before any of that. One fact for the anxious human first: FIV is strictly a cat virus. Cornell states there is no evidence it can infect people, and none that it infects dogs.

How FIV actually spreads, and how it does not

Cornell's summary is blunt: the primary mode of transmission is bite wounds from an infected cat. Casual, non-aggressive contact, sharing water bowls, mutual grooming, does not appear to be an efficient route. Sexual contact is not a significant route in cats. Mothers only rarely pass it to kittens. The adds the environmental half: the virus dies quickly outside the body, so it does not travel on hands, clothes or surfaces.

This is why the risk profile reads the way it does: the classic -positive cat is an unneutered male who roams and fights. In a UK shelter study, male cats were dramatically more likely to be infected, with an odds ratio of 27. Which also names the real protective factor: neutering. A neutered cat that does not fight over territory or mates has removed most of its exposure, and every source's advice starts there, not with isolation.

Neutering is the single change that shrinks that risk, because it shrinks the territory and the fights (how and when).

The study that should retire a myth

In 2014, researchers followed cats in two rescue facilities where -positive and FIV-negative cats lived together with, in the study's own words, unrestricted access to each other, shared litter boxes, food, water, dishes and bedding. The score after a median of 28 months: 130 uninfected cats, 8 infected housemates, zero transmissions. Nineteen kittens born to five FIV-positive mothers in the same cohort all tested negative too.

Cornell's guidance says the same thing in policy form: cats in households with stable social structures, where housemates do not fight, are at little risk of acquiring . The American retrovirus guidelines draw the practical conclusion that mixed FIV households carry minimal transmission risk.

One honest nuance rather than a smoothed-over consensus: the RSPCA in the UK, in the same paragraph where it calls transmission risk low, still recommends adopting -positive cats into single-cat households. That is a legitimate institutional risk-tolerance choice, and UK adopters will meet it, but it is a policy position layered on top of a low measured risk, not a different set of facts.

And the contrast that this site keeps flagging because the internet keeps blurring it: this friendly cohabitation picture is the story only. What FIV does over a lifetime

The course runs in three phases. An acute phase a month or three after infection: fever, swollen lymph nodes, low appetite, often mild enough to be missed entirely. Then the asymptomatic phase, which is the heart of the story: the virus replicates slowly and the cat looks and feels healthy, for months, years, and in a substantial share of cats, in the words of a 2025 review, for their entire lifetime. Only some cats reach the progressive phase, where immune function erodes and problems arrive, mostly not from the virus itself but from the secondary infections it lets in.

The late-stage picture, when it comes, tends to involve the mouth above all, chronic gum and mouth inflammation, plus recurring respiratory and skin infections, weight loss, and an elevated cancer risk: infected cats developed tumors at about five times the rate of uninfected cats in the research literature, lymphoma most commonly.

How long do FIV-positive cats live? The honest spread

Cornell's headline is the one most owners need: cats with commonly live average life spans, so long as they are not also infected with . Measured studies fill in the nuance, and the population studied is the number. In western Canada, 1,205 client-owned cats were tested and the survival of FIV-positive cats was not significantly different from matched negative cats. An Italian study at a referral clinic, where cats are by definition sicker, found FIV-positive cats reaching a median around five and a half years after diagnosis versus nearly eleven for seronegative patients there, and cats carrying both FIV and FeLV surviving barely two and a half months. That last figure is the one that justifies every dual-testing protocol.

So the fair summary: a healthy -positive cat is not a dying cat, an FIV diagnosis warrants better care rather than despair, and a double retrovirus diagnosis is a genuinely serious matter.

Testing, and the kitten trap

testing looks for antibodies, and that single fact explains its quirks. The in-clinic ELISA test can be run during a visit, positives should be confirmed by a second method, and the European guideline adds that positives in low-risk cats should always be laboratory-confirmed. It takes two to six months after infection for antibodies to rise, so a very recent exposure can test falsely negative.

The kitten trap is the antibody logic running in reverse: infected mothers hand their antibodies, not usually the virus, to nursing kittens, so a kitten can test positive for months while carrying nothing. Cornell states that few of these kittens are or will become infected. A young kitten's positive test is a schedule for retesting, not a verdict. This, incidentally, is the claim that gets misattached to testing all over the internet; it belongs here, with the antibody test.

One more wrinkle where vaccination history matters: vaccinated cats produce antibodies that older tests could not tell from infection. Australia, where the vaccine remained on sale, validated a point-of-care kit that distinguishes them with 97.1% sensitivity and 100% specificity in its trial.

The vaccine that mostly disappeared

The vaccine is a four-market story with three different endings. In the United States and Canada it was withdrawn around 2016-2017 and no FIV vaccine has been commercially available since. In the UK, and Europe generally, there never was one: the and RSPCA both state plainly that no FIV vaccine is available there. In Australia the vaccine remained available, and its record is the reason nobody mourns it elsewhere: an Australian field study measured only a 56% protective rate, and a New Zealand field study went further, finding no evidence of protection at all, with the infection rate in vaccinated cats actually higher than in unvaccinated cats in its sample.

Prevention that does work is unglamorous: neuter, keep cats from fighting, and test. Keeping an -positive cat indoors protects both the neighborhood's cats and the cat itself.

Living with an FIV-positive cat

The management plan is mostly excellent ordinary care, turned up one notch. Cornell's version: wellness visits at least every six months, annual bloodwork and urinalysis, prompt attention to any infection, parasite control, and no raw meat, raw eggs or unpasteurized dairy for an immune-compromised animal. Keep vaccinations against other diseases current. There is no cure for itself, and supportive care is genuinely effective at keeping the quiet years quiet. Antiviral research exists, interferon omega is licensed for cats in some markets and experimental compounds have reduced viral loads in studies, but nothing on that list is a routine treatment plan; that conversation belongs to your vet.

The shelter story, because adopters will meet it

For decades, a positive test in a US shelter often meant automatic euthanasia; one humane society describes exactly that policy standing until 2008. A shelter-sector magazine reports that New Hampshire law barred adopting out FIV-positive cats until 2017, and Kansas until 2019. The science above dismantled the rationale, and the sector followed: in a 2022 survey of US shelters, 64% now offer FIV-positive cats for adoption, though nearly half still keep euthanasia as an option and practices vary widely. The Australian clinical guidelines state the modern position flatly: euthanasia of healthy FIV-infected cats should not occur.
